Notes: <br /> A. On November 23,2005,the EHD issued the Notice and Order 05-01 with the compliance schedule to Forward Inc.for <br /> the violations of PRC 44004 Significant Change,PRC 44104 Terms and Conditions ot'Permit,and'127 CCR 21600 <br /> Report of Disposal Site Information. The compliance date was December 23,2005. <br /> Notice and Order 05-01 is expired. Due to non-compliance status of the facility,the issues were referred to the San <br /> Joaquin County District Attorneys Office for further enforcement and resolution. <br /> B. The concentrations of methane gas measured were less than 5%methane by volume of air at the site boundary as <br /> follows: <br /> In GPI 1: 0.1 % in shallow 0.1 %in middle 0.1 % in deep <br /> In GP12: 0.1 %in shallow 0.1 %in middle 3.3% in deep <br /> This Facility is now in compliance with the Amended Notice and Order 03-01. <br /> C. Records indicated 620 vehicle'trips occurred on 616106. The facility closed early at 5 PM to avoid violating the SWFP <br /> Permitted Traffic Volume of 620 Trips per Day. Records also indicate that 8,656 tons wac received on 617/07. The <br /> facility closed early at 5:45 PM to avoid violating The Permitted Maximum Tonnage is 8.668. <br /> D. Approximately 100 trucks per-day send off soil new module construction to French Camp. <br /> E. Load checking program needs to be updated to include the requirement of the removal of the universal waste. Operator <br /> furnished training records for personnel for procedures of handling U-wastes. <br /> F. Observed drivers of refuse vehicles wearing safety vests for hard had when they are out of their vehicle at the disposal <br /> arca(Previous AC Cleared). <br /> G. Area of ponding at north side of deck at closed site has been regraded(previous AC cleared). <br /> H.
