Forward maintains asbestos disposal manifests and operating records. An example <br /> of the operation record is included in Appendix M. The operation record includes <br /> the quantity and date of each ACW shipment, the disposal location, a summary <br /> report of any incidents which require implementing the contingency plan, results <br /> of inspection required by 14 CCR 17898.19 (b)(2)(B) and training records as <br /> required by 14 CCR 17897.19 (c)(2)(B). The notices required by 22 CCR 66268.7 <br /> (a)(11) (A-B) are retained at Forward's sales office in Stockton, along with a copy of <br /> the Hazardous Waste Manifest. All hazardous waste (ACW) manifests are <br /> retained onsite for five-years. Forward will make these records available to the LEA <br /> upon request. <br /> Forward meets the existing requirements of 27CCR for financial responsibility for <br /> closure, postclosure, and operating liability. Forward provides certificates <br /> documenting compliance wit these requirements as requested by the LEA and <br /> CIWMB. <br /> 6.4.8.4 ASH DISPOSAL METHODS <br /> The Forward Landfill accepts ash for disposal. In order to process a request for the <br /> disposal of ash, one or more of the following must be submitted: <br /> • Analytical results from a state-certified laboratory for: <br /> (a) Title 26 metals, total and soluble <br /> (b) Electrical conductivity <br /> (c) pH <br /> • Laboratory Chain-of-Custody forms. <br /> • A fully completed waste characterization form, signed by the generator. <br /> The ash is disposed directly in the appropriate WMUs, following the general <br /> landfill operation procedures discussed previously. Ash piles are spread within <br /> the WMUs with a bulldozer or a front-end loader. <br /> Forward employees working within the ash disposal area include only the dozer <br /> and water truck operators. These personnel wear protective clothing and air <br /> purifying respirators.
