4.0 RECORDKEEPING <br /> 4.1 RECORDKEEPING <br /> HWEP record files must be maintained and kept on site at all times. Load <br /> checking documentation records include, but are not limited to, the following <br /> information: (1) date of occurrence, (2) time of inspection, (3) location, (4) <br /> name of hauling firm or other information for vehicle identification (i.e., vehicle <br /> license plate number, vehicle description, etc.), (5) type of prohibited waste <br /> identified, if any (6) brief summary of the incident including source of waste (as <br /> indicated by the driver) resolution, and (7) name of load checker. Addresses <br /> and telephone numbers will be requested. In addition, load checking personnel <br /> will maintain a list of customers who repeatedly attempt to dispose of <br /> prohibited wastes in the municipal waste stream. These names will be turned <br /> over to the EA for appropriate action.
