Violation T27CCR21600 RDSI has not been amended to include the information listed in T27CCR21600(b)(6)(B),the ADC <br /> usage and waste to cover ratio. <br /> a. Submit application and RDSI amendment to include all information listed in T27CCR216000(b)(6)(B)to LEA for <br /> approval. <br /> b. Also see inspection dated 4/20/05,5/25/05,6/24/05,7/27/05,8/26/05,9/29/05, 10/28/05, 11/28/05 and 12/28/05 <br /> for previous violation of this section. <br /> c. The proposed JTD amendment received on August 2nd,2005 was rejected by this LEA in a response letter dated <br /> August 23`d,2005 due to conflicting with existing approvals for the facility. <br /> 4PAC T27CCR20580 Inadequate lighting.ghtmg. There were 2 lights in working condition. The active face area was dark and <br /> foggy. <br /> a. Provide adequate light for personnel who work at or near active face area. <br /> b. Ensure that all lights are in good working condition. <br /> 4. AC T27CCR20650 There were several ponding on the fill surface. The cover of the yesterday active face was so rough <br /> that it could promote ponding. <br /> a. Regrade the fill surface to promote lateral runoff of precipitation and to prevent ponding. <br /> b. All covers shall be graded to prevent ponding. <br /> 5. AC T27CCR20680 Exposed waste. Lots of waste was on the surface. Some waste was not completely covered by <br /> ADC. <br /> a. Cover all waste daily. <br /> b. Provide adequate cover as specified in T27CCR20680 and 20690. <br /> 6. AC T27CCR20690 The tarp used as ADC has 2 big holes. The waste was exposed in through those holes. The green <br /> waste destined for ADC had lots of household waste. <br /> a. Repair or replace those tarps. <br /> b.
