AC T27CCR 20919.5—Explosive Gas—The facility has explosive gas at the boundary above 2.5%.The following <br /> perimeter gas wells were monitored.The following results were noted. <br /> GP 11 (S) 0,8%CH4 GP 12 (S) 0.0%CH4 <br /> (I) 0.1% CH4 (I) 0.0%CH4 <br /> (D) 3.4% CH4 (D) 3.4%CH4 <br /> The facility operator needs to take the necessary steps to control the landfill gas at the boundary.Any changes or <br /> modifications to the gas extraction system must have prior approval from the SJVA PCD and the EHD.Review the <br /> Operations and Maintenance(O&M)manual to determine if all the required maintenance was conducted.Evaluate <br /> the O&M manual to determine if maintenance in adequate. <br /> T27CCR 20686-Beneficial Reuse—Processed Green Material(PGM)stockpiles prior to processing are above 131 <br /> degrees F. During the inspection the temperatures were documented at 170 degrees F. Monitor the stockpiles for <br /> temperature and maintain a log of grinding intervals. <br /> 3. AC T27CCR 20590—Health and Safety—The truck drivers unloading at the working face are not wearing the <br /> required safety vest and hard hats.During the inspection several drivers were without a safety vest and some others <br /> were without a hardhats.The company drivers for BND were the drivers that not comply. <br /> 4. AC T27CCR20860—Traffic Control—The vehicles that use the site are quelling on Austin Road.in the early morning <br /> hours prior to the facility being open. The entrance road to the facility could accommodate three rows of waiting <br /> vehicles.The entrance road had one line for a good portion of the road that morning. <br /> a. Place signage or provide clearly marked lanes on the entrance road to indicate approved area's to quell. <br /> 5. Notes: <br /> A. The working face was covered with a tarp measuring 150x100 ft.The types of cover used were soil,TASW and <br /> tarps. <br /> B. Lighting was adequate.The site added one light tower. <br /> C. Onsite litter was minimal. <br /> D. All other previous items list on March 14,2007 has been resolved. <br /> 1, OR SIGNATURE: DATE: k-7 <br />
