Robert McClellon -- <br /> )ITE NAME: Forward Inc. <br /> RECEIVED BY <br /> NSPECTION DATE: 5/24/07 OPERATOR <br /> ;OMMENTS: <br /> Violation T27CCR 20686(b)—Beneficial Reuse—The facility has been receiving the following materials as beneficial <br /> reuse. <br /> 1. Co-generation Ash <br /> 2. Sewage Sludge <br /> 3. Waste Tires <br /> 4. Street Sweepings <br /> 5. Elder Creek Waste <br /> The materials listed above have been reported as being beneficial reused.On numerous occasions the waste <br /> materials were taken to the disposal area and place directly into the working face as disposal.The facility operator <br /> 0 i has provided records showing that billing status of the waste is now being considered disposal. The facility <br /> categorized street sweepings and solid waste from Elder Creek Transfer Station as soil.These two materials are <br /> neither soil or approved,for use as ADC.Waste tires can be considered ADC if they meet the particle size.The <br /> facility operator has chosen to re-categorize the waste tires as disposal,but it is not clear that the facility has went <br /> back and made the changes to reflect those waste as overused or incorrectly reported. <br /> Title 27 CCR states that "the owner or operator shall revise the applicable reports to reflect the overuse as <br /> disposal,and pay the required Board of Equalization(BOE)disposal tipping fees for the amount of overuse." - <br /> AC T27CCR 20690—Alternative Daily Cover—The tarps used to cover the active face were torn.The torn tarps left <br /> waste exposed. The facility has purchased new tarps for use as ADC,but continue to use the existing torn tarps with <br /> out repair.This issue will be raised to a violation if documented on the next inspection report. <br /> Properly repair or replace the torn tarps prior to use of tarps. <br /> AC T27CCR 20740—Equipment —During the inspection the tipper safety bar was in disrepair and about to come off <br /> of the tipper. Properly inspect equipment for safety defects prior to using the equipment in daily operations. <br /> Notes: <br /> A. The working face was covered with a tarp measuring 60x230 ft.The types of cover used were soil,TASW,waste <br /> tires and tarps. <br /> B. Health and Safety—A few of the site users did not where the required safety vest and hardhat. <br /> C.
