STATE OF CALIFORNIA California Integrated Waste <br /> y. <br /> :;IWMB 03 (Rev. 4/04) Management Board <br /> a a <br /> pECTION REPORT ADDENDUM PAGE 2 OF 4 <br /> 4GENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br /> 31TE NUMBER: 39-AA-0015 INSPECTOR: Robert McClellon <br /> 31TE NAME: Forward Inc. <br /> RECEIVED BY <br /> NSPECTION DATE: 10/23/07 OPERATOR <br /> 'OMMENTS: <br /> 1.AC T21CCR 20590–Personnel Health and Safety–The spotter on the working face did not have a visible flashing <br /> beacon. The area where the spotter was working did not have adequate lighting. The location of the lighting towers <br /> was to far away from the working face and was not able to provide'adequate light. Place the light towers as close as <br /> possible to the working face where the spotter is working. Some of the site users were not wearing the appropriate <br /> health and safety vests. <br /> Notes: <br /> A, The active working face is located in the FU-06. The cover used was TASW and soil. The site had adequate cover. <br /> Using one large active face alleviated the traffic congestion listed as an area of concern on the previous inspection <br /> report. <br /> B. The following gas migration compliance wells were monitored during the routine inspection: <br /> GP 9(S)0.0% CH4 GP 11 (S)0.0%CH4 GP 12(S)0.0%CH4 <br /> (I) 0.0% CH4 (I)0.0% CH4 (1)0.0%CH4 <br /> (D)0.0% CH4 (D)2.4% CH4 (D)0.0%CH4 <br /> The facility monitoring records indicated that the gas migration compliance wells were below the 2.5%level during <br /> the month of September. <br /> C. The MSW transfer trucks leaving the facility were not tarping the trailers prior to leaving the facility. The un-tarped <br /> transfer trucks are contributing to the roadside litter.Make sure that all the transfer truck drivers know that they are <br /> required to trap the empty trailers prior to leaving the facility. This will be noted as an area of concern o n the next <br /> inspection report if not properly addressed. <br /> D. The site had a large number Sea Gulls at the working face. The facility needs to take steps to control the number of <br /> birds at the site. <br /> E. Onsite and offsite litter noted during the inspection. <br /> F. The site did not have adequate lighting at the working face. The facility did add a light tower,but the location of the <br /> light tower was to far from the active face to benefit the site personnel and site users.Place the light towers as close <br /> as possible to the area where the spotter is working and will be conducting early morning load checks. <br /> OCTOR SIGNATURE: ` ka—1 DATE: <br />
