TATE OF CALIFORNIA California Integrated Waste <br /> i 03 (Rev. 4/04) Management Board <br /> CTION REPORT ADDENDUM PAGE 2 OF 3 <br /> 3ENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br /> TE NUMBER: 39-AA-0015 INSPECTOR: Robert McClellon <br /> TE NAME: Forward Inc. <br /> RECEIVED BY <br /> SPECTION DATE: 11/21/07 OPERATOR <br /> DMMENTS: <br /> AC T27CCR 20830—Litter Control—MSW transfer trucks leaving the facility was not tarping the trailers prior to <br /> leaving the facility.The un-tarped transfer trucks are causing the roadside litter.Make sure that all the transfer truck <br /> drivers trap the empty trailers prior to leaving the facility. This will be noted as a violation if not resolved by the <br /> routine inspection of the facility. <br /> AC T27CCR 20810—Vector and Bird Control—There are areas on the south side of the Forward Landfill that are home <br /> to burrowing. Take steps to eliminate the vector harborage and repair the intermediate cover in those areas. <br /> 0s: <br /> A. The active working face is located in the FU-06. The cover used was TASW,PGM,soil and tarps. The site had <br /> adequate cover. The site was using two working faces. One for MSW and the other for special waste(soil,bottom <br /> ash and treated wood). <br /> B. A small amount of Treated Wood Waste(TWW)was found in the green waste pile.Make sure that TWW gets <br /> removed for the pile prior to grinding.Document all TWW that is found in the pile as part of the load-checking <br /> program. <br /> C. The following gas migration compliance wells were monitored during the routine inspection: <br /> GP 9(S)0.1% CH4 GP 11 (S)0.4%CH4 GP 12(S)0.0%CH4 <br /> (I) 0.2% CH4 (1)0.1% CH4 (I)0.0%CH4 <br /> (D)0.1 % CH4 (D)0.0% CH4 (D)0.4%CH4 <br /> The facility monitoring records indicated that the gas migration compliance wells were below the 2.5%level during <br /> the month of November. <br /> D. Onsite and offsite litter noted during the inspection. <br /> E. The site did have adequate lighting at the working face. The.facility repositioned the light towers to provide more <br /> light at the working face. <br /> H.
