STATE OF CALIFORNIA California Integrated Waste <br /> CIWMB 03 (Rev. 4/04) Management Board <br /> INSPECTION REPORT ADDENDUM PAGE 2 OF 3 <br /> AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br /> SITE NUMBER: 39-AA-0015 INSPECTOR: Robert McClellon <br /> SITE NAME: Forward Inc. <br /> RECEIVED BY <br /> INSPECTION DATE: 12/21/07 OPERATOR `— <br /> COMMENTS: <br /> L AC T27CCR 20650—Grading of Fill Surfaces—There were several areas on the top deck of FU05 and 06 that were <br /> allowing water to pond. Grade the top deck and remove low areas in order to allow runoff of the storm water by <br /> 0/15/08. <br /> 2. AC T27CCR 20690—Alternative Daily Cover —There were three large tears in the synthetic tarps used to cover the <br /> active face. Immediately repair or replace all torn tarps. <br /> 3. AC PRC 44014(b)-Terms and Conditions of the Permit—Condition"r"of the LEA conditions states"The pre- <br /> processed treated auto shredder waste must be deployed in a manner that does not allow the mixing of the cover and <br /> the waste. The pre-processed treated auto shredder waste cannot be used during rainy weather or anticipated rainy <br /> weather."The area experienced a rain event that week and the facility had areas of waste covered by TASW. <br /> 4. Notes: <br /> A. The active working face is located in the FU-06.The cover used was TASW,PGM,soil and tarps.The site was <br /> using two working faces.One for MSW and the other for special waste(soil,bottom ash and treated wood). <br /> B. A small amount of Treated Wood Waste(TWW)was found in the green waste pile. Make sure that TWW gets <br /> removed for the pile prior to grinding. Document all TWW that is found in the pile as part of the load-checking <br /> program. <br /> C. The following gas migration compliance wells were monitored during the routine inspection: <br /> GP 9(S)0.0% CH4 GP 11 (S)0.0%CH4 GP 12(S)0.0%CH4 <br /> (1) 0.0% CH4 (1)0.1% CH4 (I)0.0%CH4 <br /> (D)0.0% CH4 (D)0.0% CH4 (D)2.2%CH4 <br /> The facility monitoring records indicated that the gas migration compliance wells were below the 2.5%level during <br /> the month of November. <br /> D. Onsite and offsite litter noted during the inspection. <br /> H. The following Area's of Concern, listed during the inspection of November 21,2007 have been resolved. <br /> 1.
