STATE OF CALIFORNIA California Integrated Waste <br /> ClW B 03 (Rev. 4/04) Management Board <br /> IN TION REPORT ADDENDUM PAGE 2 OF 4 <br /> AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department <br /> SITE NUMBER: 39-AA-0015 INSPECTOR: Robert McClellon <br /> SITE NAME: Forward Inc. <br /> RECEIVED BY <br /> INSPECTION DATE: 09/28/07 OPERATOR <br /> COMMENTS: <br /> 1. AC T27CCR 20686—Beneficial Reuse—The following issues were found during the inspection. <br /> a. TASW was stockpiled with the active face,there is no separation of cover and stockpile. <br /> b. TASW was being used on the outer slopes.Normally the outer slopes should receive soil(intermediate <br /> cover). <br /> c. The green waste pile had a large amount of treated wood waste in it. <br /> 2.AC T27CCR 20580—Lighting—The site did not have adequate lighting at either working face. The lighting was an <br /> issue last winter. In order to avoid a violation on the next monthly inspection the site must obtain an additional light <br /> tower for use when needed or as a backup. <br /> 3. T27CCR 20870-Hazardous Waste—Load Checking at the working face was not being conducted. The waste was <br /> not being load checked in the early morning. The lack of proper lighting would not allow an adequate load check to <br /> be conducted. <br /> 4. AC T27CCR 20860—Trak Control—The site was using a confined work face and the morning rush of vehicles made <br /> the small unloading area unsafe. The area did not have adequate lighting,which added to the unsafe condition.The <br /> site needs to employ some type of systematic traffic controls when using a small working face to reduce the number <br /> of vehicles attempting to unload at the same time. <br /> 5. AC T27CCR 20919.5—Explosive Gas Control The following gas migration compliance wells were monitored during <br /> the routine inspection: <br /> GP 9(S)0.0% CH4 GP 11 (S)0.0%CH4 GP 12(S)0.0%CH4 <br /> (I) 0.0% CH4 (I)0.0% CH4 (I)0.0%CH4 <br /> (D)0.0% CH4 (D)2.3% CH4 (D)0.0%CH4 <br /> The facility records reflect that on August 9,2007 gas probe 11 deep registered greater than 2.5 percent methane. <br /> The facility operator needs to take the necessary steps to control the landfill gas at the boundary.Any changes or <br /> modifications to the gas extraction system must have prior approval from the SJVAPCD and the EHD.Review the <br /> Operations and Maintenance(O&M)manual to determine if all the required maintenance was conducted. Evaluate <br /> the O&M manual to determine if maintenance in adequate. <br /> 2. Notes: <br /> A. The site was using two working faces. One at the old Forward site and one at the Forward/Austin Road site. The <br /> cover used at the Forward/Austin Road site was TASW. The site had adequate cover. The outer slopes were <br /> covered with TASW. Twelve inches of soil should be used for areas that will not receive waste for 180 days. <br /> NSPECTOR SIGNATURE: �..��X�x�--- DATE: 1-7-1 <br />
