The original <br />COC torn is placed in the shipping container and the shipping container- is scaled <br />prior to being given to the carrier. The carbonless, copy of the COC firm is scat to the <br />field file. The carrier waybill will serve as an extension of the COC record between <br />the final field custodian and receipt in the laborat(-,)fy- <br />• Upon receipt in the laboratory, the quality control (QC) coordinator or a desigilated <br />representative opens the shipping containers, compares the contents with the COC <br />record, and signs and dates the record. Any discrepancies are noted on the COC fbi-iii, <br />• If discrepancies occur, the SaTTIPICS in question are segregated firom norTnal sarnple <br />storage and the field personnel immediately notified. <br />• COC records are maintained with the project records by becoming part of the <br />data/report package. <br />In addition to initiating the COC fOrni., field personnel are responsible for uniquely Identit'villp <br />(required on the foram) and labeling samples, providing proper preservation, arid packaging <br />samples to preclude breakage during shipment. <br />Lvery sample label includes the following: <br />• A unique sample number <br />• Sample Location <br />• Sampling date and 11111c <br />• Name of'person collecting the sanij)le <br />• Method of sample preservation./conditioning <br />Shipping containers are scaled at the ends with tape prior to shipment. <br />The [trial step in providing information to the labor"Itory is the request for analysis- The request <br />for analysis section is cornpieted by the field personnel on the COC record, The request for <br />analysis Ibriii is provided so that analytical requirements are defined arid sample holding tunes <br />are not exceeded. A copy of the COC and request for analysis fomi is maintained In the field <br />dalasheet/log. <br />7.2,2 Laboratory Sample Handling <br />The first step in the laboratory receipt of samples is to complete the log -in form.
