EMERGENCY CALLBFS SHUTDOWN STATUS/EVENT REPORT <br /> This report must be filled out for any shutdown occurrence prior to departure from the site. <br /> SITE: Forward Landfill JOB NO 07207048.00 <br /> 1. DATE:_03-13-2010 TIME: 4:00 pm <br /> 2. ALARM CALL-OUT YES_X_ NO <br /> 3. NAME OF INVESTIGATING TECHNICIAN: Jonathan Siva <br /> 4. ARRIVAL DATE AND TIME:_3:00 pm <br /> 5. REASON FOR MAINTENANCE/REPAIR OR INSPECTION DOWNTIME: <br /> PRE-PROGRAMMED LFG BLOWER SHUTDOWN— <br /> Pre programmed shutdown of the LFG Conveyance blower due to a value in excess of the controller <br /> operating parameters. Automatic shutdown valve closed as programmed isolating all LFG in piping <br /> system;therefore no emissions occurred as a result of this event. Shutdown occurred after early <br /> indication of system failure detected by monitoring or parametric indicators,thus preventing non- <br /> compliance. <br /> 6. DESCRIBE TYPE OF CORRECTIVE ACTION TAKEN AND EQUIPMENT/MATERIALS <br /> USED IN REPAIRS (IF NECESSARY)All operating systems were inspected prior to restart of flare <br /> station. Upon completion of inspections; system was restarted and proper operation restored. <br /> 7. RECOMMENDATIONS: <br /> 8.
