<br /> Comments:The EHD staff used the Lantec Gem 2000 to measure the concentration of explosive gas at the
<br /> property boundaries.The concentrations of methane gas by volume in SG-11 were 41%in Shallow,36.5%in
<br /> Intermediate and 26.4%in Deep.The concentration of methane gas in SG-9,SG-10 and SG-12 were 0%by
<br /> volume.
<br /> 1.Ensure that the concentration of the explosive gas is less than 5%by volume.
<br /> 2.Within 60 days of dectection(June 3,2010), implement a remediation plan that has been approved by the
<br /> EHD and the CalRecycle(formerly CIWMB).
<br /> 3.The concentration of explosive gas at the property boundary has exeeded 5%by volume every month since
<br /> May,2010.
<br /> 4.On September 28,2010, CalRecycle had notified the operator that this facility had been Included on the
<br /> Inventory of Solid Waste Facilities Which Violate State Minimum Standard for the violation of the gas monitoring
<br /> and control standard.
<br /> 5.On October 14,2010,the EHD had issued Notice and Order 2010-01,Compliance Schedule For Forward
<br /> Landfill, to the operator.
<br /> 6. The contractor Is installing new gas extraction wells as described in the 2010 Gas System Improvement
<br /> October 2010. The installation of Soil Vapor Extraction(SVE)wells will begin December 1,2010,according to
<br /> the contractor. The SVE wells shall not be installed without EHD oversight. The operator or contractor shall
<br /> contact EHD prior to installing SVE wells

<br /> 1. Notice and Order 2010-01,Compliance Schedule,was issued on October 14,2010.
<br /> 2.'According to the operator,tarps were used as ADC about 2 weeks.
<br /> 3.At time of Inspection,tarps,TASW and soil were used for cover.
<br /> 4.At time of inspection,the contractor were installing gas extraction wells in the lined units.
<br /> 5.The operator employs falconeer to control bird issues. The falconeer is on site from September 27,2010 to April 10,
<br /> 2011.
<br /> 6.According to the operator, module FU 010 began receiveing waste on Monday,November 15,2010.
