Mr. Robert McClellan <br /> October 1 , 2010 <br /> Page 8 <br /> The Forward Landfill lies near the Stockton Municipal airport and therefore, Federal Aviation <br /> Administration (FAA) clearance must be obtained prior to the start of drilling activities. All <br /> appropriate paperwork and notices shall be filed with the FAA and the Stockton Municipal <br /> airport prior to work beginning. <br /> All proposed probe locations will be located and marked using a Global Positioning System <br /> (GPS) instrument prior to drilling activities to ensure the final probe locations meet the 1,000- <br /> foot spacing requirement, and to obtain accurate elevations. Probe depths will be adjusted, as <br /> necessary, based on the elevations obtained by GPS. <br /> HEALTH AND SAFETY CONSIDERATIONS <br /> Work on or near landfill sites warrants special precautions to mitigate potential combustible gas <br /> hazards, worker exposure to the trace constituents of LFG, or exposure to physical hazards that <br /> may be encountered. <br /> The LFG extraction wells are large diameter wells, and shall be barricaded and/or covered when <br /> open. If personnel need to be within 10 feet of an open borehole, a lanyard system tied to an <br /> immovable object shall be used to prevent accidental falls. Every attempt shall be made to <br /> complete each well prior to leaving the site so that no unattended boreholes are present. <br /> A health and safety (H&S)plan for the well installation activities will be prepared prior to <br /> beginning the work. The H&S plan will describe potential hazards and will outline site-specific <br /> protection, monitoring and record keeping provisions. <br /> REPORTING <br /> An installation and evaluation report that documents the LFG extraction and SVE well <br /> installation and the evaluation monitoring will be submitted to the LEA. An installation report <br /> that documents the drilling activities, final LFG probes, and well completion details will also be <br /> prepared for submittal to Forward, Inc. Boring logs and well completion diagrams will be <br /> submitted as part of the installation report. All reports will be prepared by or under the <br /> supervision of a California Professional Geologist or Engineer. <br /> IMPLEMENTATION SCHEDULE <br /> The new LFG extraction and SVE wells will be installed as soon as Forward can obtain <br /> approvals and the necessary permits. The following is a proposed schedule for the installation, <br /> but is contingent on the Work Plan review and approval turn around time. <br />
