(hereafter Discharger) submitted a Work Plan for Monitoring
Well Installations and Abandonments to relocate monitoring wells AMW- 23, AMW-23S,
AMW-24, AMW24S and AMW-26 that are currently installed within the footprint of the planned
California Health Care Facility. These wells were originally installed as part of the ongoing
evaluation monitoring program that is required under Cleanup and Abatement Order
R5-2008-0714. To accommodate the construction schedule, all five wells must be
decommissioned prior to construction activities. The Discharger has proposed new locations
and completion depths for these wells. Only three of the five wells will be reinstalled, since the
new locations should be along the edge of the plume, rather than in the middle of the plume.
The work plan states that all wells will be abandoned, logged, installed and developed
following all applicable regulations and under the direct supervision of a California
Professional Geologist.
The work plan is acceptable and is hereby approved. Once the proper permits have been
secured by the Discharger, the field work may proceed. To document the modifications to the
monitoring network, the Discharger shall submit a well abandonment and well installation
report that includes the information listed in the enclosed Requirements for Monitoring Well
Installation Reports. This report is due no later than 30 June 2011.
If you have any questions regarding this review, please contact me at 916-464-4679.
r
HOWARD HOLD, P.G.
Engineering Geologist
Title 27 Compliance and Enforcement
Enclosure: Requirements for Monitoring Well Installation Reports
California Environmental Protection Agency
cf%Recycled Paper
