FORWARD LANDFILL <br /> SITE OCCURRENCE FORM <br /> DATE: 11/7/11 <br /> RE: BND DRIVER #351 <br /> WHAT TYPE OF OCCURRENCE: Accident <br /> CONDITIONS: Dry, clear and flat dirt surface area. <br /> 2:15pm on November 7, 2011 <br /> INSPECTOR'S NAME: RUBEN RAMIREZ <br /> DESCRIPTION OF WHAT HAPPENED: A third party driver backed his <br /> truck onto the landfill Tipper, he exited the cab to disconnect the trailer <br /> from the truck (the driver remained on steps of his truck) The truck and <br /> trailer rolled forward and the driver attempted to re-enter his cab of the <br /> truck from the step and fell off the truck step. The driver died from his <br /> injuries when the truck and trailer rolled over him. <br /> WHERE AUTHORITIES CALLED TO SITE: YES <br /> IF YES, WHO: 9-1-1 Fire Department, Ambulance <br /> ROOT OF CAUSE: The emergency brake was not engaged causing the <br /> truck to roll off the tipper. <br />
