Hazelton Ave <br /> Stockton, CA 95205-6232 <br /> RE: FORWARD LANDFILL (SWIS NO. 39-AA-0015) <br /> SOLID WASTE FACILITY PERMIT APPLICATION PACKAGE (DECEMBER 2013) AND <br /> JOINT TECHNICAL DOCUMENT (DECEMBER 2013) <br /> Dear Mr. McClellon: <br /> SWT Engineering (SWT), on behalf of Forward Inc., is pleased to submit a Solid Waste Facility <br /> Permit (SWFP) Application Package (December 2013) and Joint Technical Document (JTD) <br /> for the Forward Landfill (FL), prepared in accordance with California Code of Regulations, <br /> Title 27 (27 CCR), Sections 21570(f) and 21665. <br /> This submittal includes the following items: <br /> 1. The completed/signed SWFP application form with the "RFI/ROWD/JTD Amendment" Box <br /> 6 checked (as instructed); <br /> 2. The completed/signed Environmental Health Department Service Request form; <br /> 3. One bound, hard copy of the JTD (dated December 2013), which includes the Preliminary <br /> Closure/Post-Closure Maintenance Plan (PCPCMP) Amendment No. 4 as Appendix A to <br /> the JTD; <br /> 4. One CD-ROM containing an electronic copy (PDFs) of the JTD (as well as the PCPCMP as <br /> Appendix A); and, <br /> 5. Landfill Capacity Survey Results (provided by Geo-Logic Associates) in accordance with 27 <br /> CCR, Section 21570(1) on CD-ROM. <br /> It should be noted that payment in the amount of $625 has been paid online via credit card <br /> by Forward Inc. to cover the application fee (see attached payment receipt). <br /> Additionally, one copy of revisions to the JTD text are shown in Track Changes format (i.e., <br /> red strike through and underline) to reflect edits made addressing your review comments in <br /> letters dated August 6, 2013 and November 20, 2013.
