Preparation of the Final Report, documenting the project construction CQA activities, was the <br />last task to complete the CQA program for the Forward Landfill WMU FU -13 composite liner <br />system. This report includes discussions of the findings and observations of the on-site CQA <br />personnel regarding the activities identified above. The documentation of construction and <br />CQA activities are presented on the daily and weekly field reports prepared by our CCW <br />personnel as well as the minutes of the weekly progress meetings along with various testing <br />and observation sheets. Also presented in the appendices of this report are the results of <br />laboratory conformance tests conducted on earthworks and geosynthetic materials. The report <br />contains a discussion of the CCW procedures performed by GLA; a discussion of the <br />construction activities; and a description of the composite liner system components observed <br />and tested by GLA. The list of appendices presented in this Final Report are as follows: <br />• Design modifications to the construction documents and drawings are included in <br />Appendix A; <br />• Daily and Weekly Reports of construction and CCW activities and the Preconstruction <br />Meeting and Progress Meeting minutes are included in Appendix B; <br />• Earthworks documentation is included in Appendices C through G; <br />• Geosynthetics documentation is included in Appendices H through M; • <br />6 Geo -Logic <br />ASSOCIATES <br />Final Construction Quality Assurance Report— WMU FU -13 <br />Forward Landfill <br />1. <br />A typographical error was corrected in the Geomembrane Specification, Section <br />. <br />3.4.c.3.b.iii which read 2 psi per mil of liner thickness when it should of read % psi per mil <br />of liner thickness; <br />2. <br />The normal stress requirement at a shear stress of 2,000 psf was reduced from 700 psf to <br />600 psf to allow for a broader range of geomembrane manufacturers to be utilized while <br />maintaining an adequate factor of safety for stability; <br />3. <br />A memorandum was provided by the Design Engineer, Lewis Engineering, describing <br />revisions to the construction documents including the Geogrid Specification, the CQA <br />Manual, Sheet 3 of the Construction Drawings, and the Bid List; <br />4. <br />Section M on Sheet 9 of the Construction Drawings was modified to extend the <br />geomembrane over the anchor trench three feet rather than go into the trench; <br />5. <br />The leachate pipe and tank perforations were changed from 1/2" to 3/8" in diameter; <br />6. <br />The coarse gravel grain size distribution was modified in conjunction with the change in <br />pipe and tank perforation size discussed above; <br />7. <br />A variance was granted to utilize a higher ground pressure dozer than specified over the <br />liner system; and <br />8. <br />The use of geocomposite in place of geotextile when the supply of geotextile ran short. <br />3.4 <br />FINAL REPORT <br />• <br />Preparation of the Final Report, documenting the project construction CQA activities, was the <br />last task to complete the CQA program for the Forward Landfill WMU FU -13 composite liner <br />system.
