Preparation of the Final Report, documenting the project construction CQA activities, was the
last task to complete the CQA program for the Forward Landfill WMU FU -13 composite liner
system. This report includes discussions of the findings and observations of the on-site CQA
personnel regarding the activities identified above. The documentation of construction and
CQA activities are presented on the daily and weekly field reports prepared by our CCW
personnel as well as the minutes of the weekly progress meetings along with various testing
and observation sheets. Also presented in the appendices of this report are the results of
laboratory conformance tests conducted on earthworks and geosynthetic materials. The report
contains a discussion of the CCW procedures performed by GLA; a discussion of the
construction activities; and a description of the composite liner system components observed
and tested by GLA. The list of appendices presented in this Final Report are as follows:
• Design modifications to the construction documents and drawings are included in
Appendix A;
• Daily and Weekly Reports of construction and CCW activities and the Preconstruction
Meeting and Progress Meeting minutes are included in Appendix B;
• Earthworks documentation is included in Appendices C through G;
• Geosynthetics documentation is included in Appendices H through M; •
6 Geo -Logic
ASSOCIATES
Final Construction Quality Assurance Report— WMU FU -13
Forward Landfill
1.
A typographical error was corrected in the Geomembrane Specification, Section
.
3.4.c.3.b.iii which read 2 psi per mil of liner thickness when it should of read % psi per mil
of liner thickness;
2.
The normal stress requirement at a shear stress of 2,000 psf was reduced from 700 psf to
600 psf to allow for a broader range of geomembrane manufacturers to be utilized while
maintaining an adequate factor of safety for stability;
3.
A memorandum was provided by the Design Engineer, Lewis Engineering, describing
revisions to the construction documents including the Geogrid Specification, the CQA
Manual, Sheet 3 of the Construction Drawings, and the Bid List;
4.
Section M on Sheet 9 of the Construction Drawings was modified to extend the
geomembrane over the anchor trench three feet rather than go into the trench;
5.
The leachate pipe and tank perforations were changed from 1/2" to 3/8" in diameter;
6.
The coarse gravel grain size distribution was modified in conjunction with the change in
pipe and tank perforation size discussed above;
7.
A variance was granted to utilize a higher ground pressure dozer than specified over the
liner system; and
8.
The use of geocomposite in place of geotextile when the supply of geotextile ran short.
3.4
FINAL REPORT
•
Preparation of the Final Report, documenting the project construction CQA activities, was the
last task to complete the CQA program for the Forward Landfill WMU FU -13 composite liner
system.
