Final Construction Quality Assurance Report — WMU FU -13 <br />Forward Landfill <br />• 6.0 POLYETHYLENE PIPE, TANKS, AND FITTINGS <br />6.1 RECEIVING INSPECTION <br />Upon arrival by truck shipment, GLA's CQA Monitor performed the receiving inspection of the <br />HDPE piping, which consisted of: <br />• observing that the pipe and tanks were the proper size and dimensions in conformance <br />with the specified perforations; <br />• visually inspecting the pipe and tanks for any damage that may have occurred during <br />transportation and handling; <br />• observing and documenting that storage of the pipe and tanks was in accordance with the <br />project specifications; and <br />• documentation required by the specifications was received. <br />Documentation for the HDPE pipe and tanks is included in Appendix N. <br />6.2 INSTALLATION <br />GLA's CQA Monitor observed that the pipe and tanks were installed at the locations specified in <br />the Construction Drawings, and no damage occurred during the placement and backfill around <br />and over the pipe and tanks. GLA's CQA Monitor spot -monitored the fusion welding operations <br />to ensure that the welding followed the project Specifications. <br />In addition to the pipe, GLA's CCIA Monitor observed that the installation of the HDPE tanks <br />were as specified in the Specifications and Construction Drawings and per the approved design <br />modifications. Observation and testing of the soils around the sump, including the termination <br />berm, was done as part of the subgrade preparation and engineered fill construction. <br />31 Geo -Logic <br />ASSOC IATF.
