This Quarterly Progress Report <br /> has been prepared to provide the information specified in Task 6.a of the CAO. <br /> The information summarized in this Quarterly Progress Report has been compiled from a <br /> number of sources, including Forward, Inc. personnel, consultants responsible for the operation <br /> and maintenance (O&M) of the Unit's groundwater extraction and treatment system, and <br /> consultants responsible for the 0&M of the facility's landfill gas extraction and destruction <br /> system. <br /> All Work Completed During Previous Calendar Quarter to Comply with CAO <br /> During the period of October 1 through December 31, 2015 the following work was completed <br /> to comply with the CAO and other corrective action requirements: <br /> ➢ The Quarterly Progress Report for the Third Quarter 2015 was submitted to the <br /> RWQCB on October 14, 2015, in accordance with Task 6.a. of the CAO. <br /> Violations of the CAO <br /> CAO Task 6.a indicates that Quarterly Progress Reports shall include a description of any <br /> violations of the CAO during the relevant quarter.
