The placard- <br /> ing regulations begin in 49 CFR 172.500. <br /> 12.4.3 Manifest Requirements <br /> r <br /> DHS requires generators to complete a hazardous waste manifest for <br /> each shipment of hazardous waste that leaves the generator' s facility <br /> (Title 22, CAC, 66480-66493) . The uniform manifest provides the mech- <br /> anism for "cradle-to-grave" tracking of hazardous wastes. Among other <br /> -- things, the manifest must contain the generator' s name, address, phone <br /> number and EPA identification number; the name and EPA identification <br /> _ number of each transporter; the name, address and EPA identification <br /> number of the designated facility which will receive the waste, and an <br /> alternate facility to receive the waste, if any; and a description of <br /> the type and quantity of each hazardous waste. A sample manifest is <br /> attached in Appendix F. <br /> The generator must complete the generator and hazardous waste section <br /> of the manifest, sign it and obtain the initial transporter's signa- <br /> ture and acceptance date on the manifest. Two copies must be retained <br /> _ by the generator; the remaining copies will be given to the trans- <br /> porter. The generator is required to submit a copy of the manifest to <br /> DHS within 30 days of shipment. <br /> After the wastes reach the TSDF, the operator of that facility must <br /> sign a copy of the manifest and send it to the generator to confirm <br /> that the shipment has reached the proper destination. If the genera- <br /> tor has not received a copy of the manifest from the receiving facili- <br /> ty within 35 days of the date when transportation was initiated, the <br /> _ generator must contact the transporter and/or TSDF operator to deter- <br /> mine the status of the waste. If the signed copy is not received <br /> within 45 days, the generator must file an Exception Report with DHS. <br /> This report includes a legible copy of the manifest and a letter <br /> _r <br /> PJ9 9390502E.00D 12-9 Rev. 1 11/09/88 <br />
