Natalia Subbotnik2ya [EHL - <br />From: Michael Kith [EH] <br />Sent: Tuesday, May 03, 2011 4:32 PM <br />To: Robert McClellon [EH]; Natalia Subbotnikova [EH] <br />Subject: FW: CVWS Timeline <br />-----Original Message ----- <br />From: Hummel, Sheri [mailto:SHummel@wm.com] <br />Sent: Tuesday, May 03, 2011 4:21 PM <br />To: Michael Kith [EH] <br />Subject: CVWS Timeline <br />Michael, <br />Per our conversation the following is a preliminary timeline of the information we have <br />obtained in the past few hours regarding the possible contaminated load that may have been <br />transferred to a class III landfill. <br />At approximately 1 pm on May 3, 2011 customer service of WM received a call from Dawn <br />Tegan a residential customer who was asking for her paperwork regarding a hazardous waste <br />load she had Central Valley Waste Services pick up at her residence on April 27,2011. The <br />Resident/Customer (Dawn Tegan) alleges that CVWS picked up a 10 yard roll -off box of <br />house -hold garbage contaminated with possible hazardous waste on April 27, 2011. Customer <br />claims that earlier in March she spoke with a WM customer service representative who <br />accepted paperwork for the contaminated load and scheduled the delivery, pickup and <br />ultimate disposal of her waste at Forward Landfill. CVWS began investigation immediately <br />upon receiving the call from customer service. The tickets generated for this address were <br />reviewed, at which time it was discovered that the load was dumped at Lodi Transfer <br />Station and then transported by a third party hauler either Tiger Lines or Ruan to North <br />County Landfill. The delivery ticket identified that the load may contain hazardous <br />waste. The load was picked up on April 27, 2011, no indication of hazardous waste was <br />noted on removal ticket. At that time WM Environmental was notified and further <br />investigation is being conducted. Currently CVWS has left message with the customer for <br />further information and with the customer service representative to obtain paperwork that <br />may have been received from the customer. A call to notify the Local Environmental Agency <br />was placed on May 3, 2011 at 3:14pm. <br />I will keep you up to date with all of our correspondence and information gathered <br />regarding this incident.
