Identify potential sources of impacts and transport mechanisms <br />ii. Discuss evidence that detection monitoring program is or is not <br />working. <br />5) In Event of Release: <br />i. Same as H.2.e.3) above. <br />Preparation of contaminant contour maps for representative <br />constituents/parameters <br />The information above (H.2.a through H.2.e) shall be provided in the main body <br />of the report, and the information below (H.2.f) in the appendix to the report. <br />f. Appendix Items <br />1) Field logs of Standard Observations <br />2) Sample collection information for each monitoring point: <br />i. Time of water level measurement; <br />ii. Type of pump (or other device) used for purging and the elevation of <br />the pump intake relative to the elevation of the screened interval; <br />iii. Method of purging (the pumping rate; the equipment and methods <br />used to monitor field pH, temperature, and conductivity during <br />purging; the calibration of the field equipment; results of the pH, <br />temperature, conductivity, and turbidity testing; and the method of <br />disposing of the purge water) to remove all portions of the water that <br />was in the well bore while the sample was being taken; <br />iv. Type of pump (or other device) used for sampling, if different than the <br />pump or device used for purging; and <br />v. A statement verifying that the sampling procedure was conducted in <br />accordance with the SCAnP. <br />3) Field logs, chain of custody, and laboratory test sheets. <br />4) Copies of other relevant reports or data (e.g., results of soil gas/LFG <br />monitoring required by Local Enforcement Agency) <br />5) An electronic copy of the monitoring report on compact disk (CD) in <br />(preferably combined) PDF format. <br />
