The existing underground irrigation system shall be removed according to the <br />amended RDSI, Appendix D dated October, 1990. <br />7. All abandoned wells must be destroyed with a cement sandy slurry to the <br />bottom of well. All abandoned septic tanks must be destroyed. All <br />abandonments must be completed under San Joaquin County Public Health <br />Services permits. <br />PROVISIONS <br />1. The permit is subject to review by the Local Enforcement Agency, and may be <br />modified, suspended, or revoked, for sufficient cause after a hearing. <br />2. The use of 60 mil high density polyethylene (HDPE) liner as described in the <br />existing Waste Discharge Requirements #91-021 dated January, 1991 has <br />been approved by the Regional Water Quality. Control Board, the Local <br />Enforcement Agency, and the California lntegrated Waste Management Board. <br />The liner shall consist of'6 inches of compacted soil overlain by HDPE synthetic <br />liner. <br />3. Each waste module shall have a dendritic leachate collection and removal <br />system directly over the HDPE synthetic liner. <br />4. NCSL shall comply with the monitoring and reporting program of the California <br />Regional Water Quality Control Board. <br />CLOSURE &. POSTCLOSURE MAINTENANCE <br />1. All documentation relating to the preparation of the closure and postclosure <br />maintenance costs shall be retained by the operator and shall be available for <br />inspection by the LEA or the CIWMB. <br />2. The preliminary closure and postclosure plans were approved by CIWMB on <br />October 31, 1991 No. 91-39. <br />3. The Final closure and postclosure plans must be submitted to the LEA, Regional <br />Water Quality Control Board, and the CIWMB two years prior to closure of the <br />landfill. <br />SELF - MONITORING <br />1. Environmental measurements of water quality, leachate, and gas shall be <br />reported to LEA on a annual basis. <br />
