r] <br />An analysis request sheet or purchase order will accompany the samples <br />delivered to each laboratory. These forms will serve as official <br />communication to the laboratory of the particular analysis(es) re- <br />quired for each sample and provide evidence of the chain -of -custody. <br />A copy of the request sheet will be returned to EMCON.
