The record sheet was signed, timed, and dated by the <br /> sampler when samples were transferred. Custody transfers were recorded <br /> for each individual sample; for example, if samples had been split and sent <br /> to more than one laboratory, a chain-of-custody record sheet would have <br /> ' accompanied each sample. The number of custodians in the chain of pos- <br /> session was kept to a minimum. A copy of the Sampling and Analysis <br /> Chain-of-Custody Record was returned to EMCON Associates with the <br /> analytical results. <br /> Ground-water Sampling and Analysis Request Form <br /> A Ground-water Sampling and Analysis Request Form (see Figure A-4) <br /> accompanied the samples delivered to the laboratory.
