..V 4 9 STATE OF CALIFORNIA CIWMB 3 (Rev. 4/04) INSPECTION REPORT ADDENDUM California Integrated Waste Management Board I9_Lei�•=i79 AGENCY CONDUCTING INSPECTION: San Joaquin County Environmental Health Department SITE NUMBER: 39 -AA -0003 SITE NAME: Harney Lane Landfill INSPECTION DATE: May 4, 2007 COMMENTS: INSPECTOR: Garrett Backus RECEIVED BY , OPERATOR?—A-- A routine quarterly inspection was done on this day. 1) Violation — T27 CCR §20919.5 — Explosive Gas Control — Perimeter landfill gas monitoring well GW -1 contained 7.4% methane in the middle probe. Within seven days of detection, place in the operating record the methane gas levels detected and a description of the steps taken to protect human health. 2) Area of Concern — T27 CCR §20750 — Site Maintenance — A landfill gas extraction pipe has two low spots which will allow condensation to plug the pipe, impeding the flow of landfill gas. The pipe is located on the east slope of the secondary site. Repeat AC from last quarter. Repair the pipe slope to remove the low spots in the pipe. 3) Area of Concern — T27 CCR §20750 — Site Maintenance — A landfill gas (LFG) extraction pipe has a disconnected coupling. The pipe is located on the east slope of the secondary site. Repair the pipe and coupling connection. 4) Area of Concern — T27 CCR §20530 — Site Security - The rear gate near the flare station was open. Keep this gate closed, especially overnight, to prevent unauthorized access to the site. 5) Note — The fence along the southeast corner of the property was repaired since the last inspection. Previous AC is resolved. 6) Note — The following perimeter landfill gas monitoring probes were measured for % methane: ID: shallow, middle, deep probes GW -1: 0.0%, 7.4%, 0.3%;GW-2: 0.0%, 0.0%, 0.0%;GW-3: 0.0%, 0.0%, 0.7%; GW -4:1.3%,0.7%, 0.0%;GW-11: 0.0%, 0.0%, 2.2%. 7) Note — The landfill gas extraction flare was operating.
