DIRECTIONS FOR FILLING OUT CIWMB-56 FORM <br /> Fill out inspection form completely. Total hours/Time In-Time Out is optional,but recommended. Mark the lines to the right of each law or regulation with one of the <br /> following codes: <br /> V=Violation A=Area of Concern NA=Not Applicable <br /> All unmarked laws/regulations indicate compliance. Summarize inspection results in the COMMENTS section. If additional space is required, use form SWIS-03. Include <br /> explanations for determinations of NA when appropriate. Check form(s) for completeness/accuracy and submit to the California Integrated Waste Management Board within <br /> 30 days of the inspection in accordance with 30 PRC 43218. <br /> PARTIAL SUMMARY OF APPLICABLE LAWS AND REGULATIONS <br /> Consult Title 14,California Code of Regulations,and the Public Resources Code for the complete laws and regulations listed on this form. <br /> PRC 44004-No significant change in design or operation except in conformance with approved solid waste facilities permit(SWFP). Application filed with LEA for <br /> modifications in design or operation of facility at least 120 days in advance of date proposed modification to take place. <br /> TITLE 14,CCR <br /> CLOSED SITES-SITES SUBJECT TO 7.8 STANDARDS <br /> 17765-Treatment units located within the property boundary of a landfill incorporated in closure and postclosure maintenance plan. 17766-A written postclosure <br /> emergency response plan prepared and maintained and contains required information. 17767-A sign at all points of access has been posted at site sixty days prior to <br /> closure. Within ten days after receipt of the final shipment of waste,all points of access to the site are restricted to authorized personnel. Sign posted at main point of <br /> access within ten days of receipt of final shipment of waste indicating location of closure/postclosure plans and telephone number for emergency notification. <br /> 17772- Components of environmental control systems,which have come in contact with leachate or landfill gas,decommissioned. 17776-Site designed with final <br /> grades. Differential settlement calculations provided at specified frequency. 17777-The operator ensures the integrity of final slopes. Slopes steeper than a horizontal to <br /> vertical ratio of one and three quarters to one(57%or 30°)supported by slope stability report. Fifteen foot wide bench for every fifty feet of <br /> vertical <br /> height. <br /> 17778-Final drainage system designed,constructed,graded,and maintained. Run-on and Run-off control systems and collection facilities maintained. <br /> 17779- Procedures developed and implemented to protect the integrity of the final cover. 17781 -Procedures to monitor,collect,treat and dispose of leachate. <br /> Landfill Gas: 17783.5-Perimeter subsurface monitoring wells installed and located at or near the property boundary or alternate boundary. 17783.7- On-site structures <br /> monitored. 17783.9-Monitoring probes and on-site structures sampled during the monitoring period. 17783.11 -Monitoring conducted at least quarterly unless alternative <br /> frequency approved by LEA and Board. 17783.13-Monitoring results submitted to LEA and Board within ninety days of sampling. 17783.15-Corrective action <br /> developed and implemented when gas monitoring indicates concentrations of methane in excess of 1.25%within on-site structures and/or 5.0%at the property boundary. <br /> 17783.17-Landfill is exempt from gas monitoring and control requirements during closure and postclosure. <br /> 17787-Detailed description of site filed with County Recorder, LEA,Board and local agency selected to maintain the county solid waste management plan. <br /> 17788-Landfill maintained and monitored for a period of 30 years after completion of closure. 17796-Proposed construction on completed sites submitted to LEA and <br /> Board for review and comment. <br /> 18275- Certification of closure submitted to the LEA and Board. <br /> SITES UNDERGOING CLOSURE: <br /> 17763-Closure schedule implemented within thirty days of final shipment of waste. Change in schedule submitted to LEA for approval. 17768-LEA and Board notified <br /> within ten days after last shipment of waste received. 17771 -Structures removed or secured,monitored and maintained. 17772-Components of environmental control <br /> systems,which have come in contact with leachate or landfill gas,dismantled and disposed or cleaned if intended for reuse. 17773-A minimum thickness and quality of <br /> cover material in place. 17774-Construction quality assurance program. <br />
