Health and Safety Plan Consent Agreement <br /> Because of the potentially hazardous nature of this site and activity occurring at the site, it is not <br /> possible to discover, evaluate, and provide protection for all possible hazards that may be <br /> encountered. Strict adherence to the health and safety guidelines set forth in this document will <br /> reduce, but may not eliminate, the potential for injury and illness at this site. Guidelines in this <br /> Plan were prepared specifically for this site and should not be used on any other site without <br /> prior evaluation by trained health and safety personnel. <br /> Site workers must also review this HASP. The Site Safety Officer (SSO) must conduct a pre- <br /> work briefing prior to initiating this project. All sections of this HASP must be reviewed during <br /> this briefing. Any worker not attending the initial meeting must be trained by the SSO on the <br /> information covered in the pre-work briefing meeting. The SSO must hold tailgate meetings at <br /> the beginning of each work shift to discuss important safety and health issues concerning <br /> tasks to be performed on that day. After reading the HASP and attending a pre-work briefing, <br /> workers must sign the following acknowledgment statement. <br /> I have read, understand, and will abide by the information set forth in this HASP.
