Notes: <br />RECEIVED BY <br />OPERATOR <br />176 <br />T14 CCR 17408.6 - Maintenance - Area in front of transfer station building has worn and cracked asphalt and <br />potholes. The EHD has not received any repair time schedule regarding permanent repair of asphalt. <br />a. Per conversation with operator on June 8, 2006, operator requested permanent repairs to be postponed to May 2007. <br />Postponing the repairs will require adequate interim measures until such time permanent repairs are made. <br />Otherwise, permanent repairs shall be completed prior to this coming rainy season. <br />b. Submit repair time schedule for permanent repair of asphalt. Include a description of interim measures (if repair is <br />to be postponed to May 2007). Description shall include of any changes of the operation with a site plan to include <br />location of tipping area & traffic patterns. <br />T14 CCR 18221.6 — TPR - U -waste screen program is currently in place. <br />a. Submit updated information regarding u -waste screening operation currently in place. <br />T 14 CCR 17408.1 — Litter - Small amount of off site litter observed at north east corner gate. <br />a. Remove litter. <br />All other previous AC's cleared. <br />A. Low waste pile volumes were observed at time of inspection. <br />B. A meeting was held with Central Valley Waste Services on April 4, 2006 regarding proposed permanent u -waste <br />dismantling operation on-site. It is understood that a draft description of this operation will be submitted to the EHD <br />for review (prior to submitting a formal RFI amendment application package). This proposal shall be evaluated for <br />a significant change determination. Per conversation with operator on May 8, 2006, submittal of this information is <br />pending. <br />C.
