<br />Fill out inspection form completely. Mark the lines to the right of each law or regulation with one of the following codes:

<br />Summarize inspection results in the COMMENTS section. If additional space is required, use form SWIS-03. Include explanations for determinations of N/A or N when
<br />appropriate. Check form(s) for completeness/accuracy and submit to the California Integrated Waste Management Board within 30 days of the inspection in accordance
<br />with 30 PRC 43218.
<br />PARTIAL SUMMARY OF APPLICABLE LAWS AND REGULATIONS
<br />Consult Title 14, California Code of Regulations, and the Public Resources Code for the complete laws and regulations listed on this form.
<br />PRC 44004 -No significant change in design or operation except in conformance with approved solid waste facilities permit (SWFP). Application filed with LEA for
<br />modifications in design or operation of facility at least 120 days in advance of date proposed modification to take place.
<br />LARGE VOLUME
<br />17441 -Amendments to RSI filed to keep information current. 17442 -Enforcement agency notified of significant changes in the design or operation of the station.
<br />17451 -Design of new facility shall utilize expert advice. 17452 -Station design shall be based on appropriate data, take account of features needed to accomodate public use.
<br />17453 -Station designed to minimize nuisances and detrimental effects to public health and the environment. 17461 -Accurate weight/volume records maintained in a
<br />manner and form approved by the LEA. 17462 -Log of special occurrences maintained (fires, injury and property damage accidents, explosions, incidents regarding
<br />hazardous waste, flooding, and other unusual occurrences). 17463 -Records available for inspection.
<br />17471 -Adequate number of qualified personnel available. 17472 -Personnel adequately trained. 17473 -Adequate supervision provided. LEA and appropriate fire control
<br />authority advised of names, addresses, and telephone numbers of operator, station manager, and site supervisor. 17474 -Attendant available full-time or as determined by
<br />LEA.
<br />17481-I.D. signs at each point of access indicating name of station operator. Other information may be required by LEA. 17482 -Entry sign located at appropriate point
<br />states fees, hours of operation, wastes accepted or not accepted. 17483 -Adequate perimeter security. Hazardous areas fenced and identified.
<br />17484 -Roads have smooth surface, minimize dust and tracking of mud, and allow all-weather access.
<br />17485 -Visual screening provides attractive appearance as approved by LEA and local land use authority. 17486 -Waste bins are durable, easily cleanable, safe and leak-
<br />proof. Tipping areas safe, easily cleanable, and designed to prevent the loss of waste. All station equipment in good condition and cleaned in frequency and manner to
<br />prevent vectors and other nuisances.
<br />17491 -Adequate sanitary facilities available at station or immediate vicinity. 17492 -Safe adequate drinking water available.
<br />17493 -Communication facilities adequate. 17494 -Adequate lighting as approved by LEA.
<br />17495 -Sufficient fire fighting equipment available and located as required by local fire authority and LEA. 17496 -Contact between users and wastes minimized. Barriers
<br />provided as necessary. 17497 -Safety equipment in use and being worn as required by LEA.
<br />17511 -Unloading adequately confined. Wind-blown materials adequately controlled. 17512 -Daily cleaning of loose materials and litter, or as approved by LEA.
<br />17513 -Wastes removed every 48 hours or as approved by LEA. 17514 -Uncleaned or fully loaded transfer vehicles must not be parked on public streets or roads except
<br />under emergency conditions.
<br />17515 -Scavenging prohibited. 17516 -Salvaging must not interfere with operations. 17517 -Volume reduction operations minimize health, safety, and nuisance problems;
<br />does not interfere with operations. 17518 -Salvaging and volume reduction confined to clearly identified areas. 17519 -Processing operations are conducted in an organized
