This <br />letter constitutes notice, pursuant to Public Resources Code, <br />Section 44015, that the permit issued to the above facility is <br />due for permit review. <br />An application for permit review, including an updated Report of <br />Facility Information, should be filed with the Local Enforcement <br />Agency by September 23, 1994. Upon completion of the review, the <br />Local Enforcement Agency will determine if a revision or <br />modification of the permit is required. If a revision or <br />modification is necessary, you will be directed to submit the <br />appropriate Solid Waste Facility Permit application. <br />If you feel this notice is in error, please notify us in a letter <br />addressed to my attention by September 9, 1994. Please include <br />the facility name, number and the original date of permit <br />issuance. Also include the most recent review date-, and a brief <br />explanation of why a review of the permit is not required at this <br />time. <br />Your cooperation is appreciated.
