> SOLID WASTERCILITY PE acility Number:IT 39-AA-0008 16. LEA Conditions: 1. This facility shall comply with all terms and conditions of this permit. 2. This facility shall comply with all applicable State Minimum Standards for Solid Waste Handling and Disposal. 3. This facility shall comply with all Federal, State,and Local requirements,and enactments including all mitigation measures given in any certified environmental document filed pursuant to Public Resources Code,Section 21031.6 4. Any additional information concerning the design and operation of this facility must be provided upon request of the Local Enforcement Agency (LEA). 5. Household Hazardous Waste shall be handled in a manner approved by the LEA. 6. This permit is subject to review by the LEA and may be suspended,revoked,or modified at any time for sufficient cause. 7. This facility must conform to the Report of Station Information dated September of 1993, and amended January of 1994. 8. The LEA reserves the right to suspend or modify waste receiving operations when deemed necessary due to emergency, an actual or potential health hazard, or creation of a public nuisance. 9. Any change that would cause the design or operation of this facility not to conform to the terms and conditions of the permit is prohibited. Such a change may be considered a significant change requiring a permit revision. In no case shall the operator implement any change without first submitting a written notice of the proposed change to the LEA at least 120 days in advance of that change. 10. Safety equipment shall be provided for all employees and workers at the transfer station. 11. This facility must comply with the condition of approval for Land Use Permit UP-93-2. 12. The Waste Removal Plan for Lovelace Transfer Station must be approved by the LEA and the CIWMB before excavation begins. Page 4 of 4
