This notice is issued pursuant to Title 27, <br />California Code of Regulations, Section 21675: <br />The Solid Waste Facility Permit issued to the Lovelace Transfer Station on August 16, 2005 is due <br />for permit review on August 16, 2010. <br />Please submit an application for a permit review (attached) (form CIWMB E-1-77) to the San Joaquin <br />County Environmental Health Department (EHD) within 30 days of receipt of this letter. Submit a plan <br />review fee of $575 with the review application. Submit a Service Request (SR) form (attached). Upon <br />completion of the review, the EHD will determine if a revision of the existing SWFP is required. The <br />operator requirements for submittal of an application are described in Title 27 Section 21640. In <br />summary, the application for permit review shall include: <br />• Proposed changes in design and operation; <br />• Updates to the Transfer Processing Report.
