The traps were <br /> not reset. The Pest Sighting Log showed an inspection was done on 2/12/14 and then on 2/27/14, but none in <br /> between those dates. The 2/27/14 inspection should have resulted in documenting in the Pest Sighting Log that <br /> there were two sprung traps observed with no rodents and that the traps were reset. This has been a continuing <br /> issue. <br /> 1. The operator shall take adequate steps to control or prevent the propagation, harborage, and <br /> attraction of vectors. <br /> 2. Ensure that daily inspections of the traps and bait station are done and that proper <br /> documentation is done in the Pest Sighting Log. <br /> 016n,R Gomrr►ents <br /> c <br /> Routine monthly inspection conducted)on this date and the following was noted: <br /> 1. The fire extinguishers were properly charged. <br /> 2. There were no issues with dust or odors. <br /> 3. Records, including training and tonnage with load checking were available for review on <br /> site. The training records on site were up to date. <br /> 4. There were four cats observed on site. Two were in the pile of bakery waste. The <br /> bakery waste is attracting the cats. Implement a fence to close off the doorways <br /> surrounding the building. Submit, by the next routine inspection, a written plan for <br /> installation of the fence. <br />
