Well located northwest of LISA houses has holes in the concrete surrounding the well head which <br /> could lead to ponding around the well head. Seal all holes in the concrete. Submit evidencd of <br /> corrective action by 1/20/2017. <br /> Notes: <br /> • At the time of this inspection there were approximately 302,000 birds on site. <br /> • 1 of the 2 chick houses(east house) on the west side of the facility is now in use. <br /> * Houses LISA 5, 6, and 7(free range)were inspected. House 7 is currently empty. <br /> Concentration of flies was low both inside and outside of the houses. Bait stations were <br /> observed both inside and outside of the houses. These houses are cleaned every day. <br /> 0 Inspected caged houses 1, 2, 3, and 4.Fly strips inside of houses are changed weekly. Fly <br /> bait is used as needed.Bait stations were observed both inside and outside of the houses. <br /> Houses are cleaned every day. <br /> 0 Houses utilize a manure belt system. Manure is composted in a building at the south end of <br /> the facility. <br /> • Houses are inspected daily for mortality. Mortality is incinerated daily onsite. <br /> • Fly concentration onsite was low both inside and outside of the houses. <br /> • Odors outside of the houses were low. <br />
