In the event ash is applied during any month, submit monthly ash dumping <br /> reports. Provide the amount of daily tonnage and number of trucks. <br /> 9. Obtain a written determination from RWQCB on whether a report of waste <br /> discharge and/or an application for waste discharge requirements will be <br /> required. In the event the RWQCB determines it has jurisdiction over the ash, <br /> the applicant must comply with all terms set forth by the RWQCB. <br /> 10. Provide a copy of the quarterly monitoring reports to EHD of the Wadham <br /> Energy, Inc. ash as required by the Department of Health Services letter of July <br /> 8, 1991. <br /> 11. Provide health and safety equipment to field workers who will be handling the <br /> ash operations at each site. <br /> 12. All conditions of this permit must be met prior to the beginning of operations. <br />
