OBSERVATION: The"initial training dates" listed for the employees were after the"date of assuming responsibility as a facility employee"in the Facility Employee Training Certificate dated 1/12/2020,4/8/2020, and 5/26/2020. -The initial training dates are not accurate and are the same as the date of training; several of the employees are listed as having received training on previous training logs. -The date of assuming responsibility as a facility employee is only listed as the year and not the full date. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. 1)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -The designated operator answered the question regarding sump sensor locations but did not list which sumps were visually inspected. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. 2)OBSERVATION:The designated operator failed to check and note the correct test dates for the secondary containment testing on the designated operator inspection reports. Secondary containment testing was performed on 1/22/2018. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class ll.
