SAN 10 A Q U I N Environmental Health Department <br /> ---COUNTY - <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> HOLLYWOOD CARS HOSPITAL 1042 S SINCLAIR AVE, STOCKTON September 20, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: The eyewash bottle over the sink had an expiration date of 07/2009. <br /> REGULATION GUIDANCE: All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the eyewash into proper working order by repair, replacement or testing. Provide a <br /> corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 21, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an <br /> appointment, please call (209) 468-3420. <br /> Documents provided during inspection: Return to Compliance certification, flyer for free CUPA classes, <br /> hazardous waste labels, hazardous waste label guidance, application for ID number renewal, modified <br /> contingency plan, HHW business brochure <br /> Waste streams found: <br /> -Spent Paint Booth Filters <br /> -Bondo Dust <br /> Notes: <br /> -A modified contingency plan was provided during the inspection and filled out on site. <br /> -Bondo dust is disposed of in the trash without first making a hazardous waste determination. Per Jose Luis <br /> Perez (Owner) Bondo dust and spent paint booth filters will now be managed as hazardous waste and taken to <br /> the SJC HHW Facility going forward. Corrected on site. <br /> -Spent paint booth filters have not been generated yet and will be handled as hazardous waste per above note. <br /> -Per Jose Luis Perez(Owner) no waste paint is generated. Excess paint is used as a base coat on other cars. <br /> -Facility owes$330 in permit fees.
