CONSENT FOR ENTRY AND SAM LING <br /> Permission is granted to Advanced GeoEnvnonmental, Inc (AGE), acting on behalf of Paul Sxdhu, <br /> property owner of 3032 East Waterloo Road, Stockton, Callforma, to access three monitoring wells <br /> that are part of a ground water investigation related to 1905 North Broadway Avenue, Stockton, <br /> California The three monitoring wells have been designated MW-b, MW-7 and MW-8 by <br /> Sierra-Pacific Groundwater Consultants, Inc <br /> Monitoring and sampling of the three wells will be conducted in accordance with San Joaquin <br /> County regulations to collect ground water samples as part of an investigation of a fuel release at <br /> 3032 East Waterloo Road, Stockton, California <br /> AGE will agree to hours of access designated by Unocal, Inc and Sierra-Pacific Groundwater <br /> Consultants, Inc and make reasonable efforts to avoid or minimize interference with or disrupt any <br /> business activities at the well locations AGE will hold general commercial liability insurance to <br /> cover $1,000,000 for damage, personal damage and death, auto, and Worker's Compensation in <br /> accordance with the laws of the State of California <br /> INDEMNITY <br /> . Unocal, Inc and Sierra-Pacific Groundwater Consultants, Inc have not assumed liabilities for any <br /> injury or damages that may occur due to AGE's activities in the vicinity of the monitoring wells <br /> AGE and the property owner of 3032 East Waterloo Road agree to indemnify, defend and hold <br /> harmless Unocal, Inc and Sierra-Pacific Groundwater Consultants, Inc against any claims, suits or <br /> demands, including attorney's fees and court costs, ansmg out of AGE's performance of <br /> investigation work on the monitoring wells brought by any tlurd party, person, entity or government <br /> body Also, AGE and the property owner of 3032 East Waterloo Road agree to indemnify, defend <br /> and hold harmless Unocal, Inc and Sierra-Pacific Groundwater Consultants, Inc against any claims, <br /> suits or demands, including attorney's fees and court costs, anszng out of, or in any connected to <br /> AGE's entry and sampling of the monitonng wells <br /> Authorized by <br /> Please Print Clearly <br /> Signature Date <br /> Title <br /> Company <br /> i <br />
