HARNEY LN., LODI, CA 95240 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of the application <br /> for a permit to install a Xerxes new underground storage tank (UST) system at the above noted address, <br /> which will consist of: <br /> TA0520849 20,000 gallon regular gasoline <br /> TA0520850 12,000 gallon diesel (split tank) <br /> TA0520851 8,000 gallon premium gasoline (split tank) <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local Fire District, the San Joaquin Valley Air Pollution Control District, and the local <br /> Community Development Department for applicable permits. <br /> 2. Any changes shall be submitted to the EHD for approval prior to commencing work, including <br /> change in contractors, service technicians, installers, etc. All new submittals supersede the <br /> previous ones. <br /> 3. Qualified installers must possess or work under the direct and personal supervision of an individual <br /> physically present at the work site, at all times while work is being performed, who possesses all of <br /> the following: <br /> • One of several permissible valid, current contractor's licenses issued by the Contractors State <br /> License Board (may be issued to the installation company), <br /> • A valid, current UST Installation/Retrofitting certificate issued by the International Code Council <br /> (ICC), <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> installed. <br /> 4.
