In order to complete your application you will need a copy of the unauthorized <br /> release report(UAR) and the UST removal permit. The UST removal permit is proof that the tank <br /> was in compliance with San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) permitting requirements at the time of removal. The UAR is filed when <br /> contamination is discovered at UST removal.
