12 June 1996
AGE-NC Protect No 95-0130
Iand decreased in wells MW-1, MW-3, and MW-5
It should be noted that since ground water was between 5 to 10 feet above the tops of the screen
Eintervals in monitoring wells MW-1 through MW-4 during the April 1996 monitoring, the analytical
results may not be representative of actual dissolved petroleum hydrocarbon concentrations in the
native ground water
Preliminary historical research of the former Wells Fargo Bank property, located north of the site,
has indicated that USTs may have been located on the property (see Appendix C - Sanborn Fire
Insurance Maps) Furthermore, the PHS-EHD does not have any information on record indicating
that USTs were removed from the Wells Fargo Bank property
Further assessment is required north and northeast of MW-5, on the former Wells Fargo Bank
property to determine the extent of dissolved hydrocarbons in ground water We recommend the
installation of two additional monitoring wells on the former Wells Fargo Bank property During
completion, we recommend the use of a minimum of 30 feet of screen to ensure that the ground
water does not rise above the top of the screen intervals
A Corrective Action Plan (CAP) was submitted to the PHS-EHD in March 1996 Upon regulatory
P g D
approval of the CAP, installation of a remediation system should commence
6 0. LIMITATIONS
Our professional services were performed using that degree of care and skill ordinarily exercised by
environmental consultants practicing in this or similar localities The findings were based upon
analytical results provided by an independent laboratory Evaluations of the geologic/hydrogeologic
conditions at the site for the purpose of this investigation were made from a limited number of
available data points (i e borings, monitoring wells), and subsurface conditions may vary away from
these data points No other warranty, expressed or implied, is made as to the professional
recommendations contained in this report
