Mr. Arshad, <br />I have reviewed the CERS submittal and compliance status and have found the following issues: <br />• Violation #6, the site map does not show or address the following: <br />1. A legend to identify the codes, symbols, or abbreviations used on the map was not provided. <br />2. Loading areas. <br />3. Internal roads. <br />4. Storm and sewer drains. <br />5. Evacuation staging area. <br />6. The location of the waste oil. <br />7. Emergency response equipment (fire extinguishers and spill control). <br />8. Emergency shutoffs. <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br />and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br />lr hol ric "AIA" <br />• Violation #7, Emergency Response/Contingency Plan is not complete. <br />1. Section C. the name and phone number of the nearest medical facility/hospital has not been identified. <br />2. Section G. the location of each item checked has not been identified. <br />3. Section E. element E3, the location of the Emergency Assembly Area(s) has not been identified and <br />element E4, has not been completed. <br />4. Section F. has not been completed. <br />Violations 2, 3, 6, 7, 9 and 12 remain open.
