OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. - Monitoring system certification test result of 5/21/2019 not found on site -Secondary containment test result of 08/05/2019 not found on site. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 205 CCR 2712(1), 2634(e) Failure to submit a current UST Response Plan and have it available onsite. OBSERVATION: An approved copy of the response plan was not available for inspection. REGULATION GUIDANCE: A copy of this plan shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The DO report of 4/6/2020, 1/21/2020, 10/21/2020, 7/29/2019, and 7/1/2019 did not include the owner/operator name, signature and date signed. The reports failed to add a description in section III to all components marked N/A or N from section VI I through XI. -The DO reports of 5/4/2020 and 3/9/20 did not include the owner/operator name, the reports failed to add a description in section III to all components marked N/A or N from section VII through XI. -The DO reports of 11/25/2019, 9/23/2019 and 8/27/2019 failed to add a description in section III to all components marked N/A or N from section VII through XI. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
