The incorrect and missing information included the following from the inspection reports: <br /> -The 2/10/20, 3/9/20,and 5/4/20 D.O reports did not include the owner/operator name. <br /> -The 4/6/20, 9/23/19, 8/27/19, 7/29/19 and 7/1/19 D.O reports did not include the owner/operator name, signature <br /> and date signed. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> OBSERVATION: Liquid was observed in the 91 fill and STP sumps, and 87 fill sump. <br /> REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it <br /> must be removed and disposed of properly. <br /> CORRECTIVE ACTION: The service technician removed approximately 15gallons of liquid from the 87 fill sump <br /> and approximately 2 gallons of liquid from the 91 fill and STP sumps. Ensure that all sumps and annular spaces are <br /> maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 318 CCR 2666(c), 2643(c)(1) Leak detector failed to detect a 3.0 gallons per hour leak at 10 pounds per square inch. <br /> OBSERVATION: The 89 VMI line leak detector failed to detect a leak when tested. <br /> REGULATION GUIDANCE: All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> CORRECTIVE ACTION: The service technician adjusted the leak detector, retested it, and it passed. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> FA0006441 PR0231442 SCO01 06/11/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
