The incorrect and missing information included the following from the inspection reports: <br /> -The 2/23/20, 4/30/20 and 6/18/20 DO reports left blank section VI <br /> -The 11/25/2019 DO report did not include the date signed by owner/operator <br /> -The 8/27/2019 DO report was signed on 8/30/2019. The owner/operator did not sign the DO report within the <br /> required time frame 48hours of receiving the DO report. <br /> -The DO report was performed on 2/23/20 the next report performed on 3/31/20 more than 30days after the <br /> previous inspection. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Seepage was detected below the shear valve in under dispenser containment (UDC) 7A, <br /> indicating a leak in the primary containment. <br /> REGULATION GUIDANCE: All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a minor violation. <br /> 318 CCR 2666(c), 2643(c)(1) Leak detector failed to detect a 3.0 gallons per hour leak at 10 pounds per square inch. <br /> OBSERVATION: The 91 VMI line leak detector failed to detect a leak when tested. <br /> REGULATION GUIDANCE: All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> CORRECTIVE ACTION: The service technician adjusted the leak detector, retested it, and it passed. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 8/14/2020 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> FA0003593 PR0231691 SCO01 07/15/2020 <br /> EHD 23-01 Rev.9/20/2019 Page 4 of 5 Underground Storage Tank Program OIR <br /> 1868 E.
