S0 JOA U 1 Environmental Health Department <br /> U ;� i I <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Pilot Travel Center Lathrop- 1017 345 Roth Rd, FRENCH CAMP August 25, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 313 HSC 25290.1,25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Leaking meter was observed in UDC 1/2 and small amount of liquid was observed in UDC 9/10, <br /> indicating a leak in the primary containment. <br /> REGULATION GUIDANCE:All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> On site for the monitoring system certification. The testing was not witnessed in its entirety. Looked inside all <br /> sumps and UDCs. Verified proper sensor locations. Paperwork inspection is still needed. A checklist was left <br /> on site; a complete report will be provided at the time of paperwork inspection. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
