Manifest number 003371797GBF (5/1/2017)was not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter,the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter,the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. This is a Class II violation. 114 HSC 25160.2(b)(4)Consolidated manifest receipts failed to contain complete information. The consolidated manifest receipts for used oil dated 1/25/2019 and 4/23/2018 were missing the following required information: correct ID number. The consolidated receipts have an ID number of CAL000219696 which belongs to Concrete Inc at 10260 Waterman in Elk Grove. The ID number for this site is CAL000253842. All copies of each receipt shall contain all of the following information: -The name, address, identification number, contact person, and telephone number of the generator, and the signature of the generator or the generator's representative; -The date of the shipment; -The manifest number; -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream type listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North America(UN/NA)identification number, if applicable; -The name, address, and identification number of the authorized facility to which the hazardous waste will be transported; -The transporter's name, address, and identification number; -The driver's signature; -A statement, signed by the generator, certifying that the generator has established a program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be economically practicable. Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all required information. This is a minor violation.
