The following is an itemized list of hazardous materials violations that have not been addressed for CAL TRANS MAI NT SHOP 10 as of August 19, 2020. Open violations from July 10, 2020 inspection ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #10 - Failed to submit revisions to business plan within 30 days of substantial change in operations. During the inspection, twelve cylinders of propane cylinders (estimated 152 gallons; actual volume was not available)were found on site and the updated information has not been reported. Within 30 days of one the following events, business shall electronically update the information submitted to the California Environmental Reporting System (CERS): -A 100 percent or more increase in the quantity of a previously disclosed material. -Any handling of a previously undisclosed hazardous materials -A change of business address, business ownership, or business name. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. Documentation showing initial and annual refresher training of 1. Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency; 2. Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment; and 3. Evacuation plans and procedures, including immediate notice, for the business site; was not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment (C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
