The following is an itemized list of hazardous materials violations that have not been <br /> addressed for CAL TRANS MAI NT SHOP 10 as of August 19, 2020. <br /> Open violations from July 10, 2020 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #10 - Failed to submit revisions to business plan within 30 days of substantial change in <br /> operations. <br /> During the inspection, twelve cylinders of propane cylinders (estimated 152 gallons; actual volume was not <br /> available)were found on site and the updated information has not been reported. Within 30 days of one the <br /> following events, business shall electronically update the information submitted to the California Environmental <br /> Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Documentation showing initial and annual refresher training of 1. Immediate notification contacts to the appropriate <br /> local emergency response personnel and to the unified program agency; 2. Procedures for the mitigation of a <br /> release or threatened release to minimize any potential harm or damage to persons, property, or the environment; <br /> and 3. Evacuation plans and procedures, including immediate notice, for the business site; was not available at the <br /> time of inspection. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training. All employees shall be trained in safety procedures in the event of a release or threatened release <br /> of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> Page 3 of 4 <br />
